We make it easy to schedule an appointment with one of our excellent providers. Fill out this form and we will confirm your appointment, usually within an hour or two.
You can select whether you would like to see a provider in one of our two Lafayette offices, in Durango, or a virtual online teletherapy session.

Our office staff will follow up with you as quickly as possible to confirm your appointment. They typically respond in less than 24-hours during our normal business day, or first thing the following business day.
Current Patients
You may request an appointment with your clinician using your Patient Portal messaging. If you do not have access to the portal or have not been set up yet, please feel free to call us or use this form to request an appointment.
